The application enables the provision and coordination of a wide range of integrated, holistic home care services, provided by multidisciplinary teams of health professionals and social workers.

The home care services can be provided either at home, either at Open Protection Centers or to Closed Protection Centers.

The application consists of the following components:

• Home Care Plan that is developed and monitoring by healthcare professionals of multiple disciplines.

• Scheduling of Home Visits implemented by visiting personnel of multiple disciplines

• Module of On Line Real Data Acquisition from Portable Devices for the collection of actual medical readings at home visits and their automatic integration in the patient MHR.

As well as:

• Multimedia Health Record that includes all patient related data including real medical data and is used to coordinate the home care service provision

• Diagnosis Management

• Patient Monitoring Management

• Financial data management

• Managerial Statistical Reports

• Data Security Mechanisms

• Definitions and Codification Scheme

• Capability of networking of different people or organizations cooperating in the provision of home care services, regardless of their physical location

• The coordinated provision of a multitude of health and social services at home, provided from professionals of various disciplines

• The upgrade of home care services almost to the level of hospital services resulting in the decrease of hospitalizations

• The collection of actual medical data at home via portable medical devices as well as their integration in the patient MHR.

• The central coordination of Care Plans issued and implemented by various, different professionals and the Scheduling of home visits via a Virtual Coordination Centre οf doctors.

• The increase in personnel productivity and decrease in personnel costs since home visits are implemented by paramedical and not medical personnel and

• The decrease in administrative overheads
