Frontis Workflow

 

All activities related to Home Care Service provision are directed from the Virtual Coordination Centre (VCR) that consists of medical doctors and experts of multiple disciplines as well as the head of paramedical personnel that visits patients at home,. This VCR coordinates and schedules home visits by professionals of multiple disciplines (i.e. nurses, physiotherapists, social workers, etc.).

Patient MHRs are used at the same time by all health professionals involved in home care service provision, like:

  • Administrative personnel

  • Medical doctors and experts of multiple disciplines

  • Paramedical personnel of multiple disciplines conducting home visits

  • Head of the paramedical personnel of multiple discipline who conduct home visits

  • Department of Medical Materials & Drugs Management

  • Οrganization Management

The tasks implemented by the above personnel via the system, can be divided into the following broad axes:

  • Administrative Services

  • Medical Services

  • Management of information (MIS)

Finally, given that all home care professionals work on the same MHRs, a very significant software feature that relates to data security and protection of use from unauthorized personnel, is the capability to hide or disable specific fields on MHR tabs for specific users or group of users. In this way, tasks are facilitated since personnel views only data relevant to their specific tasks, and the organization security policy is reinforced.

For example, the access of the paramedical personnel user group can be fixed by the System Administrator in such a way so that when a nurse opens the MHR, he or she sees the patient name and attendant doctor name but not other personal patient data such as address, telephone, etc., or medical data such as medical history, e.t.c.

With the Frontis system, Home Care Services can include the full cycle of healthcare service provision at home, that is:

  • Prevention

  • Continuous Monitoring

  • Therapy and Recovery

The Workflow of Home Care Service Provision is described below:

  • The prescribing doctor develops the Care Plan for the patient based on the patient's medical history and examination actual results - which he records on and / or accesses via the MHR.

  •  

  • The visiting professionals, that implement the Care Plan at home, are equipped with laptops and portable medical devices. During each visit they access the Care Plan at the Multimedia Care Record of the patient and implement the specific Care Actions as these are ordered from the prescribing doctor.

  • They collect actual medical data via their portable medical devices. This real data is automatically downloaded to the laptop and then integrated into the MHR.

  • All Care Actions implemented are logged, time stamped and integrated in the Multimedia Care Record.

  • The personnel responsible for medical materials and drugs are automatically updated via the system on the Care Actions to be implemented at home visits and the materials or drugs required and schedule their materials and drugs orders accordingly.

  • The medical experts at the Virtual Co-ordination Centre examine the implementation of the Care Plans and the real medical data collected during home visits so they acquire a perfect picture of the patient's health without having to visit the patient at home. They conduct comparisons and correlations of different types of medical results so as to effectively monitor the patients' situation without leaving their headquarters.

  • Based on the examination of the patient data as this is integrated at the Multimedia Care Record, they dictate modifications that are integrated into the patient's Care Plan and implemented in next Visiting Professionals’ home visit.

  •  

  • The Virtual Co-ordination Centre and the head of paramedical personnel schedule the Visiting Professionals' work automatically.

The system automatically processes the cost of all provided home care services and allocates them accordingly between the recipient of the services and any insurance organization that covers these services.
